Your Credentials will be as follows:
Username: lastname + month + year
Password: Lastname + YearOfBirth
For example:
Name: Mary Summers
Date of Birth: July 27, 1955
Username would be: summers071955
Password would be: Summers1955
** Please note that for the password, the last name has to be capitalized.
